As our community grows, we are continuing to refine the way we support both clients and our care management team.

Erin Judkins has transitioned into the role of Director of Care Management. In this role, she focuses on supporting the care management team, strengthening clinical collaboration, and maintaining a broader view of each client’s care journey.

As Erin moves into this expanded leadership role, Cassidy Duffy and Carol Ruesink are stepping into Senior Care Manager roles. In this capacity, they provide guidance and support to members of our care management team and help ensure that each client’s plan continues to evolve thoughtfully over time.

This structure allows every client to benefit from the expertise and close relationship they have with their primary care manager, while also drawing on the perspective of an experienced leadership team that keeps an eye on the broader picture of care.
