Who are we?
Care Management by Natalie is a team of diverse experts who focus on the whole individual. While our first priorities are to ensure the stability of each client's home and medical needs, we also prioritize each client's social engagement and preparedness for their future as well.
Care Management by Natalie Geriatric Care Managers include passionate Nurses, Social workers, and Dietitians specializing in geriatric care. Imagine an additional member of your personal support team that has the knowledge and many years of experience in the geritaric field. This experience and personal connections save time and can help connect you with the businesses and resources that best fit your needs.
Our team has personally overseen large projects such as home renovations to keep clients living independently, coordinated and assisted in cross country moves to bring loved ones closer to their support networks. We’ve helped navigate and educate clients and their families through new physical and mental diagnoses. As well as smaller tasks like attending medical appointments, picking up medications, and ensuring our clients, their caregivers, their senior community staff, and families understand and are up to speed on any changes in medical instructions.
Our team of dedicated companions can also provide assistance in keeping you independent in your home. As well as encouragement and planning to engage with friends, family, community, and hobbies that have become difficult to engage with independently. They are also a valuable resource for those living in senior communities. Assisting with engaging within their community and providing reliable access to the outside community. This team helps fill a common gap for assistance that is not at a level of requiring a caregiver. We cover the greater Washtenaw County, Brighton and Plymouth, Michigan areas.
